Direct Pay Organization
We are a direct pay organization and are Out-Of-Network with all insurances. This means we do not submit bills to insurance on your behalf. Payment for all services is due at the time of visit. We offer Reimbursify at no cost to help you easily submit out-of-network insurance claims for potential reimbursement.

Established Patient New Injury/Problem
Includes thorough medical history review, comprehensive physical exam, diagnostic ultrasound, personalized treatment plan for established patients
Follow up Visit
Includes review of progress, imaging review, adjustment of treatment plan, and additional recommendations
Sports Physical / Pre Participation Physical Exam
Includes complete physical examination and necessary forms

CorticoSteroid Injection
Price is for single injection, additional injections on the same day will be discounted to $50.
Shockwave Therapy (EPAT)
Price is for a single session for one body region. (*Minimum 3 sessions*). Additional body region on the same day will be discounted to $50. (Package of 6 = $900)
PRP Injection
This price is for single injection. Additional injections on the same day are discounted. Call to discuss pricing.

ProloTherapy
Price is for single body region, additional injections on the same day will be discounted to $100.
Trigger Point Injection
Price is for up to three muscle regions (e.g. neck, shoulders, upper back). Additional regions will be $50 per region.
